New York uses a state-based Marketplace, NY State of Health — not HealthCare.gov as the official enrollment site. Individual and family qualified health plans, the Essential Plan, Medicaid, and Child Health Plus run on different eligibility tests from Medicare Advantage or employer small-group coverage. Short-term limited-duration medical insurance is prohibited in New York.

Direct answer: Lock the county first. Then confirm the hospital and two doctors you will actually use. Then compare total yearly cost after subsidy or Medicare extra help — not the company logo. A Buffalo Independent Health network does not follow you to Queens, and a MetroPlus NYC plan is not an Upstate Excellus card.

What to verify before you enroll
- County and ZIP, not “New York” as a single market.
- The legal product: QHP, Essential Plan, Medicaid, Child Health Plus, Medicare Advantage, Part D, Medigap, or small group.
- Primary care, specialists, and the hospital you would use for surgery.
- Each maintenance drug on that contract’s formulary and tier.
- HMO / EPO referral rules versus any PPO out-of-network language.
- Premium after APTC, Essential Plan rules, or Medicare premium adjustments — plus deductible and out-of-pocket maximum.
- The current enrollment window. Individual Open Enrollment on NY State of Health has recently run November 1 through January 31; Medicare Advantage and Part D Annual Enrollment runs October 15 through December 7 for a January 1 effective date. Confirm the live calendar before you wait.
