Claims
How to file and track a Ameritas claim
For a life insurance claim, start the claim online through the Ameritas claims page or call 1-800-745-1112 to report a life insurance death claim or Care4Life accelerated death benefit claim. Have the insured’s full name, date of birth, date of death, and Social Security number available. After an online submission, Ameritas mails instructions for creating an online account. You can then upload documents and track the claim status through the life insurance claims portal.
The claims page also provides instructions for other Ameritas products. Annuity death claims may be started online or by phone. For dental, hearing, LASIK, and vision claims, contact the provider first because the provider may submit the claim. Disability customers can request claim forms securely through Ameritas Accounts or by emailing disabilityclaims@ameritas.com. Do not include personal information in that email. Customer service hours listed for service representatives are generally Monday through Friday, 7 a.m. to 5 p.m. Central Time.
Before you report a claim
- Find the policy details. Look for the policy number and the insurance company's name on the policy, statements, or bank records showing premium payments.
- Get certified death certificates. Most companies ask for a certified copy. The funeral home can usually order several.
- Contact the company's claims team. Ask for a claim form and a list of the documents they need.
- Complete the claim form carefully. Each beneficiary usually files separately. Include your contact and tax information.
- Keep copies of everything. Save the claim number and notes from every call.
What happens after you file
After Ameritas receives a complete claim, it reviews the policy, the beneficiary designation, and the documents. Claims can take longer if the policy is less than two years old, the cause of death needs review, or paperwork is missing. Customers can enroll in autopay through Ameritas Accounts using the EFT enrollment form. Mail checks payable to the applicable Ameritas entity and include the policy number. Whichever method you choose, save your billing statements and payment confirmations until the policy term ends. If you change banks or cards, update your payment details with Ameritas before the next draft date so a payment doesn't fail.
